Informed Consent Waiver, Release and Hold Harmless Service Agreement for Nutrition Counseling Services by clicking you acknowledge you are enrolling in Nutrish Mish, Inc nutrition counseling. I understand that I have been advised and it is my responsibility to check with my doctor before starting this or any nutrition program. I hereby affirm to the best of my knowledge that I do not suffer from any condition other than those disclosed to my nutritionist. I understand that Nutrish Mish, Inc is not medical advisement and the opinion of a doctor should always set precedent. In consideration of my participation in the above program, I, for myself, my personal representatives, administrators, heirs and assigns, hereby hold harmless Nutrish Mish, Inc, its Board, employees and agents, from any claims, demands, and causes of action, including reasonable legal expenses and attorney’s fees, arising from my participation in the program. I understand Nutrish Mish, Inc. has a 24-hour cancellation policy, and am aware that I will be lose a session for a missed appointment if proper notice is not given (by phone or email at least 24-hours prior to the scheduled appointment). If a payment is owed and appointment is canceled with less then 24 hours notice, a $19 fee will be charged to your account. When next package is purchased, this $19 will be subtracted from your payment owed. We respect your privacy and all information collected will be protected from all third parties. Membership Packages may not be shared or transferred.
No refunds are accepted on products or services after purchase. Packages are activated once materials have been distributed. In extenuating circumstances at NMi’s discretion in the event that a refund is warranted pro rating will always be done using single non discounted session pricing. Some packages are billed using monthly progress payments against a full package price. The billing and payment is not contingent on use. Once contracted NMi provides the necessary infrastructure to accommodate increased booking and as such will collect full payment.
Cancelations are required 24 hours before appointment. Less than 24 hours’ notice will result in loss of that session. No shows will also result in loss of a session.
Packages may be put on freeze in extenuating circumstances and require a written request which must be approved in writing. I hereby affirm that I have read, have been honest with Nutrish Mish, Inc. and fully understand the above information. I have been given the opportunity to present questions in all related matters.
